Picking up of NHF cards on behalf of the customer(s) such as the elderly, the vulnerable, and persons who don’t have the time to go and pick up the NHF cards themselves.

Requirements: Permission Letter from the prospective client, copy of customer ID, the NHF form filled out by both customer and doctor.
